As a new client, when you book online, you will receive an email with an invitation to create a clinic account.  For your convenience that invitation will contain one combined form. Please take a few minutes to fill out this form. Because the majority of clients in my practice vary the type of sessions they receive, I kindly ask everyone to fill out this comprehensive form when getting established, regardless of which session you choose, in order to streamline your care going forward.
